Beginning January 1, 2020 the Health Department Office will have new hours for Wednesday’s.
Office Hours are:
Monday , Tuesday and Thursday 8:00a.m. to 4:00 p.m.
Wednesday 12:00p.m. to 5:30p.m.
Friday 8:00a.m. to 3:00p.m
Certificate Issuance is available between the hours of 8:15a.m. to 3:45p.m. Monday , Tuesday and Thursday. Wednesday 12:00p.m. to 5:15 p.m. Friday 8:15a.m. to 2:45p.m.
Statewide Issuance for Ohio is now available at all local Health Departments for birth certificates. The Jefferson County Health Department now maintains all the birth and death records for Jefferson County.
How far back do your birth and death records go?
The Jefferson County Health Department has birth records which start in 1876 and death records beginning in 1908.
What is the cost of a birth/death certificate?
The cost for a certified birth or death certificate is $28.00 We accept cash, money order. Credit and Debit cards are accepted, there is a service fee for use of a card.

How long will it take to receive it?
Once the payment has been received, the certificate is usually sent out the next business day. You may also stop at our office and pick up your copy during normal business hours:

Information needed by the clerk
As of January 2011, the JCHD moved to statewide certificant issuance. Any birth that occurred in the state of Ohio can be issued through this office. The name, the date of birth, mother’s maiden name, father’s name needs to be included with the request.
Death certificates are still filed on where the person was pronounced deceased. To obtain death certificate, you will need to contact that health department.
All birth and death records at the Jefferson County Health Department are filed by year, then alphabetically by last names.
