Information needed by the clerk
For a birth certificate, we need to know where
the birth took place. For a death certificate,
we need to know where the person was pronounced
deceased. Next, we will need to know the
year of the birth or death.
If the person was born or pronounced deceased
inside the city limits or a hospital, those records
will be found at the Steubenville City Health
Department.
If the person was born or pronounced deceased
outside the city limits of Steubenville, those records will be
on file at the Jefferson County Health Department.
All birth and death records at the Jefferson County
Health Department are filed by year, then alphabetically
by last names.
What if there is an error on a birth or death
record?
If a child's father was not listed on the birth
record at the time of birth, a supplement to the
record can be made within the first year.
If a birth or death record contains an error,
an affidavit can be filed with the State, which
will then become an attachment to that record.
These forms can be obtained at the Jefferson
County Health Department, 500 Market Street, Steubenville,
Ohio. |
